I hereby give consent for my child to participate in the Revolutionary Youth Theatre Summer Intensive Program. I understand that the payment made to secure my child's spot in the program is non-refundable. I acknowledge that my child will be engaging in various activities as part of this intensive program, including but not limited to rehearsals, workshops, and performances, under the supervision of qualified professionals. I agree to abide by the terms and conditions set forth by Revolutionary Youth Theatre regarding participation in the program, including any rules or guidelines provided for the safety and well-being of all participants.
